ABSTRACT

SUMMARY: Temporomandibular joint dysfunction interferes with the quality of life and activities of daily living among patients. The symptoms of temporomandibular dysfunction, including pain and clicking and popping sounds, are worsened during stressful events, and patients report increased pain around the temporomandibular joint. Stress-related behaviors, such as teeth clenching and teeth grinding, are commonly reported as increasing during stress. The prevalence of temporomandibular dysfunction and stress-related behaviors is reported differently in the literature. Stress in higher education is common. The purpose of this pilot study was to investigate the prevalence of temporomandibular joint dysfunction and stress-related behaviors among staff members at a local University. The study also sought to explore pain patterns described by people experiencing temporomandibular joint dysfunction and the relationship between stress-related behaviors and pain symptoms experienced. Further, the impact of stress on symptoms experienced by people with temporomandibular dysfunction was investigated in this pilot study.


La disfunción de la articulación temporomandibular interfiere con la calidad de vida y las actividades de la vida diaria entre los pacientes. Los síntomas de la disfunción temporomandibular, incluidos el dolor y los chasquidos, empeoran durante los eventos estresantes, y los pacientes informan un aumento del dolor alrededor de la articulación temporomandibular. Los comportamientos relacionados con el estrés, como apretar y rechinar los dientes, suelen aumentar durante el estrés. La prevalencia de la disfunción temporomandibular y los comportamientos relacionados con el estrés se informa de manera diferente en la literatura. El estrés en la educación superior es común. El propósito de este estudio piloto fue investigar la prevalencia de la disfunción de la articulación temporomandibular y los comportamientos relacionados con el estrés entre los miembros del personal de una universidad local. El objetivo del estudio además fue explorar los patrones de dolor descritos por personas que experimentan disfunción de la articulación temporomandibular y la relación entre los comportamientos relacionados con el estrés y los síntomas de dolor experimentados. Además, en este estudio piloto se investigó el impacto del estrés en los síntomas que experimentan las personas con disfunción temporomandibular.

Aim: This study aimed to assess the prevalence of temporomandibular disorders (TMD) and psychosocial comorbidities in undergraduate dental students in a southern Brazilian university, during the COVID-19 pandemic. Also, it aimed to verify the association between psychosocial factors and TMD. Methods: Fonseca Anamnestic Index, Depression Anxiety Stress Scale (DASS-21), a socioeconomic questionnaire, and questions about academic performance and social distancing were applied. Poisson regression analysis was used to assess the association of predictive variables with TMD. Results: The prevalence of TMD was found to be 82.4%, and more than half of the students had some degree of stress, anxiety, and depression. Students who had symptoms of stress (RR 1.11; 95% CI 1.04-1.19), anxiety (RR 1.19; 95% CI 1.12-1.27) and reported academic performance worsening (RR 1.12; 95% CI 1.07-1.19) had higher TMD scores. Conclusion: The findings suggest that TMD was highly prevalent among dental students at a federal university in southern Brazil during the pandemic, being associated with high levels of stress, anxiety, poor academic performance, and greater social distancing

ABSTRACT Introduction: Temporomandibular disorder (TMD) refers to a group of conditions that compromise the harmonious movement and function of the temporomandibular joint, masticatory muscles, and associated structures. The etiopathogenesis of TMD is multifactorial but not well-understood, with the role of genetic factors still being unclear. Objective: This review aims to summarize the results of studies that evaluated TNF-α levels and the -308G/A TNF-α polymorphism in TMD patients. This study emphasizes the importance of a more selective treatment involving TNF-α inhibitors that can potentially reduce inflammation and pain, and improve quality of life. Methods: The MEDLINE/PubMed database, Cochrane Library, and Web of Science database were searched for case-control studies published until September 2020 that compared levels of TNF-α or presence of its -308G/A polymorphism in TMD patients and healthy individuals. Results: Six case-control studies were identified with a total of 398 TMD patients, aged between 12 and 78 years. The control group consisted of 149 subjects, aged between 18 and 47 years. The occurrence of TMD was predominant in females. Majority of studies found high TNF-α levels in TMD patients, compared to the control group. One of these studies found a positive correlation between the GA genotype and the development of TMD. Conclusion: Majority of the TMD patients showed elevated TNF-α levels, and a possible explanation for this could be the presence of the -308G/A polymorphism.


RESUMO Introdução: A disfunção temporomandibular (DTM) é definida como um grupo de alterações que comprometem a articulação temporomandibular, os músculos mastigatórios e as estruturas associadas. A etiopatogenia da DTM é multifatorial, e o papel dos fatores genéticos permanece obscuro. Objetivo: A presente revisão teve como objetivo descrever as contribuições de estudos que avaliaram os níveis de TNF-α e o polimorfismo -308 G/A em pacientes com DTM. Esse estudo enfatizou a importância de um tratamento mais completo envolvendo os inibidores do TNF-α que podem potencialmente reduzir a inflamação e a dor, contribuindo para melhorar a qualidade de vida do paciente. Métodos: As pesquisas foram realizadas nas bases de dados MEDLINE/PubMed, Cochrane Library e Web of Science, em busca de estudos de caso-controle publicados até setembro de 2020 que avaliassem os níveis de TNF-α e seu polimorfismo -308 G/A nos pacientes com DTM e em controles saudáveis. Resultados: Seis estudos de caso-controle foram identificados, com um total de 398 pacientes com DTM, e a idade variou de 12 a 78 anos. O grupo controle consistiu de 149 indivíduos e sua idade variou, aproximadamente, de 18 a 47 anos. O sexo feminino foi predominante. A maioria das pesquisas encontrou níveis elevados de TNF-α nos pacientes, em comparação com os controles. Um estudo encontrou uma associação positiva entre o genótipo GA e o desenvolvimento de DTM. Conclusão: A maioria dos pacientes com DTM demonstrou predisposição a uma maior produção de TNF-α, e isso poderia ser explicado pela presença do polimorfismo -308 G/A.

Abstract The objective of this study was to evaluate if individuals with dentofacial deformities (DFD) who require orthognathic surgery are affected more by depression and pain. A case-control study was performed with 195 individuals. In the DFD group, 145 individuals with Class II and III malocclusion requiring orthognathic surgery were selected. The control group was composed of 50 individuals with no DFD. All patients were diagnosed according to the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). Data were analyzed with a significance level of 0.05. The DFD group more often presented severe depression (p = 0.020) and chronic pain (p = 0.017). They also presented higher prevalence of Nonspecific Physical Symptoms Including Pain (P = 0.002) and Nonspecific Physical Symptoms Excluding Pain (p = 0.002). Concerning TMD symptoms, the DFD group had more myofascial (p = 0.002) and articular pain (p = 0.041). Therefore, the results of this study suggest that depression and pain are more common in individuals with DFD requiring orthognathic surgery compared with individuals without DFD.

Abstract Dentists are exposed to contamination by SARS-CoV-2 due to dental interventions, leading to a state of alert and potential risk of negative impact in mental health and sleep quality, associated with Temporomandibular Disorder (TMD) and bruxism. Objective: to evaluate the psychosocial status, sleep quality, symptoms of TMD, and bruxism in Brazilian dentists (DSs) during the COVID-19 pandemic. Methodology: The sample (n=641 DSs) was divided into three groups (quarantined DSs; DSs in outpatient care; and frontline professionals), which answered an electronic form containing the TMD Pain Screening Questionnaire (Diagnostic Criteria for Temporomandibular Disorders - DC/TMD), the Pittsburgh Sleep Quality Index (PSQI), the Depression, Anxiety and Stress Scale (DASS-21), and the sleep and awake bruxism questionnaire. ANOVA test and Mann Whitney post-test were used, with Bonferroni adjustment (p<0.016) and a 95% confidence level. Results: Probable TMD was found in 24.3% (n=156) of the participants, while possible sleep and awake bruxism were diagnosed in 58% (n=372) and 53.8% (n=345) of them, respectively. Among all variables evaluated, only symptoms of depression were significantly greater in the quarantined DSs group when compared to those who were working at the clinical care (p=0.002). Working DSs were significantly less likely (OR=0.630, p=0.001) to have depressive symptoms. Those who were not worried or less worried about the pandemic were less likely to experience stress (OR=0.360), anxiety (OR=0.255), and poor sleep quality (OR=0.256). Sleep had a strong positive and moderate correlation with psychological factors on frontline workers and DSs in outpatient care, respectively. Conclusion: The results suggest confinement may have a more negative impact on the life of DSs than the act of being actively working. The concern about Covid-19 and poor sleep quality was significantly prevalent and may negatively affect the quality of life of DSs. Thus, further research on the topic is needed.

ABSTRACT

Objetivo: Comparar clinicamente os pacientes com desordens buco-maxilo-faciais (DBMF) atendidos em uma clínica-escola de fisioterapia da cidade de Fortaleza. Materiais e métodos: Dentre 5.357 prontuários, foram coletados os dados de 315 prontuários referentes aos pacientes acometidos por DBMF, dentre eles, 62 homens e 253 mulheres. Resultados: O estudo adotou perfil predominante de pacientes do gênero feminino, solteiros, entre 21 a 30 anos, estudantes, que fizeram uso de relaxante muscular; mulheres sentiram dor irradiada, já homens, dor do tipo fina. Bruxismo/endentações/desgaste dentário foram prevalentes em ambos os sexos e os sintomas que apresentaram diferença estatística entre homens e mulheres foram cervicalgia (p=0,0051), cefaleia (p<0,0001), formigamento de membros superiores (p=0,0371) e dor corporal (p=0,0234). Conclusão: Bruxismo/endentações/desgaste dentário foram os prevalentes em ambos os sexos. Entre os homens, o sintoma mais prevalente foi a dor ou cansaço ao mastigar, já entre as mulheres foi a cefaleia.

Resumen Introducción: La disfunción temporomandibular (DTM) puede estar ocasionada por numerosos factores, como maloclusiones, traumatismos, bruxismo, así como por estrés y depresión. Esta última se caracteriza por un estado de profunda tristeza, abatimiento e infelicidad. Ambos trastornos pueden presentarse en la adolescencia, cuando se crean las pautas de lo que será la persona en la adultez, tanto en el aspecto psicológico como en el emocional y el anatomofisiológico. El objetivo de este estudio fue determinar la relación del grado de DTM con el grado de depresión en adolescentes de ascendencia maya de la comunidad de Yobaín, Yucatán, México. Métodos: Se llevó a cabo un estudio de tipo correlacional y transversal. Se incluyeron 210 adolescentes de ascendencia maya de entre 10 y 19 años de edad. Para el estudio se utilizaron el índice de Helkimo y la escala de depresión de Beck. Se utilizó estadística descriptiva e inferencial. Resultados: El 74.3% de los adolescentes presentó algún grado de DTM y el 51.9%, algún grado de depresión. Se encontró que a mayor grado de DTM, mayor es el grado de depresión. La frecuencia de DTM es similar tanto para la adolescencia temprana como para la tardía, aunque en la temprana se observó mayor frecuencia de depresión grave. Conclusiones: A falta de otros estudios similares, se logró sentar un precedente con este estudio acerca el estado de salud psicológica y odontológica en esta población de origen yucateco. Se estableció una pauta para acudir a otras áreas de atención médica, como psiquiatría, neurología y neuropsiquiatría.


Abstract Introduction: Temporomandibular dysfunction (TMD) is caused by multiple factors such as depression, which is characterized by a state of deep sadness, dejection and unhappiness that can be transitory or permanent. Both of these disorders can occur in adolescence, when guidelines are created for what the person will be like in adulthood for the psychological, emotional and anatomophysiological aspects. The objective of this study was to determine the relationship between the degrees of DTM and depression in adolescents of Mayan ancestry from the community of Yobain, Yucatan, Mexico. Methods: A correlational and transversal study was conducted, using the Helkimo index and the Beck depression scale, in which 210 adolescents aged 10 to 19 years of Mayan descent were included. Descriptive and inferential statistics were used. Results: Some degree of DTM was observed in 74.3% of the participants, and some degree of depression in 51.9%. A higher degree of DTM was associated with a greater degree of depression. The frequency of DTM was similar for both categories of adolescence: early and late. However, a higher frequency of severe depression was observed in early adolescence. Conclusions: In the absence of other similar studies, a precedent was set with this study on the state of psychological and dental health in this population of Yucatan origin. Also, a guideline was established for referral to other areas of health care, such as psychiatry, neurology and neuropsychiatry.

ABSTRACT

Objetivo: considerando-se o elevado número de pessoas com disfunção temporomandibular (DTM) e que a busca por tratamento tem aumentado nos últimos anos, o objetivo do presente estudo foi avaliar a prevalência de DTM entre os integrantes da Equipe de Saúde da Família (ESF) e o conhecimento dos profissionais em relação a essa condição. Métodos: os questionários foram aplicados em 87 profissionais da ESF do Grupo Hospitalar Conceição. No primeiro questionário, avaliou-se a prevalência de DTM, no segundo, foram coletados os dados sociodemográficos e, no terceiro e último, avaliou-se o conhecimento sobre a etiologia, o diagnóstico e o tratamento das DTMs. A análise dos dados foi realizada por meio dos testes Qui-Quadrado ou Teste T de Student para um p ≤ 0,05. Resultados: os resultados demonstraram que os profissionais incluídos, em sua maioria, apresentam DTM leve e que o percentual de acertos dos dentistas foi superior estatisticamente ao dos médicos apenas no item tratamento. Conclusão: é de suma importância que os profissionais das ESFs estejam preparados para atender a demanda de pacientes que apresentam sinais e sintomas compatíveis com DTM.(AU)


Objectives: considering the high number of people affected by TMD and the demand for treatment has increased in the last years, the objective of the present study was to assess the prevalence of TMD among the members of the Family Health Team (FHT) and professionals' knowledge regarding this condition. Methods: three questionnaires were applied to 87 professionals from the Family Health Teams of the Hospital Conceição Group. The first questionnaire evaluated the prevalence of TMD, the second one collected the sociodemographic data and, finally, the research questionnaire sought to evaluate the professionals knowledge about the etiology, diagnosis and treatment of TMD. The data analysis were performed by the Chi-square or Student's T-Test for a p ≤ 0.05. Results: the results demonstrated that the professionals included in the sample have most have slight TMD. Regarding the professional knowledge, the percentage of correct answers of the dentists was statistically superior to the one of the doctors only in treatment. Conclusion: it is of the utmost importance that FHT professionals are prepared to meet the demand of patients with signs and symptoms compatible with TMD.(AU)

Introducción: Los pacientes con trastornos mentales están sometidos a un mayor número de factores de riesgo de enfermedades bucodentales y temporomandibulares. Ello debido a los efectos secundarios de las medicaciones que consumen, la falta de autocuidado, la dificultad para acceder a atención, la actitud hacia los profesionales sanitarios y también la falta de cooperación en los tratamientos dentales. Objetivo: Determinar la frecuencia de los trastornos temporomandibulares (TTM) y el índice de dientes cariados, perdidos y obturados (CPO-D) en pacientes con discapacidad psicosocial crónica internados en el Hospital Psiquiátrico de la ciudad de Asunción, Paraguay. Material y Métodos: El estudio tuvo un diseño observacional descriptivo de corte transversal y fue aprobado por el comité de ética institucional. Participaron 139 personas con discapacidad psicosocial crónica de ambos sexos, mayores de 18 años, en el mes de mayo, 2018. Como instrumentos de medición se utilizaron: un cuestionario de 3 preguntas, el examen clínico con 5 indicadores para la variable presencia de TTM, el odontograma para el índice CPO-D y los expedientes clínicos. Resultados: Se encontró una población en su mayoría masculina, con un rango de edad entre 40-50 años, y un alto porcentaje de fumadores activos. Se observó que el 83,5 % de los examinados presentaba TTM. El índice CPO-D fue 22,8. Entre los signos de TTM, el ruido articular fue el más común. Conclusión: Se observó una elevada frecuencia de TTM en las personas con discapacidad psicosocial y un elevado índice CPO-D. Introduction: Patients with mental disorders are subject to a greater number of risk factors for oral and temporomandibular diseases. This is due to the side effects of the medication they consume, the lack of self-care, the difficulty in accessing medical help, the attitude of the patients towards health professionals and also the lack of cooperation in their dental treatments. Objective: To determined the frequency of temporomandibular disorders (TMD) and the decayed, missing and filled teeth index (DMFT) in people with chronic psychosocial disability admitted to the Psychiatric Hospital of the city of Asunción. Material and Methods: This descriptive, observational and cross-sectional study was approved by the local ethics committee. One hundred and thirty-nine people with chronic psychosocial disabilities, of both sexes, over 18 years of age participated during May, 2018. The instruments used were a 3-item questionnaire, clinical examination with 5 indicators for presence of TMD, the odontogram for the DMFT index and the clinical charts. Results:A predominantly male population, with an age range between 40-50 years, and a high percentage of active smokers was found. TMD was found in 83.5% and the DMFT index was 22.8. Among the signs of TMD, joint noise was the most common. Conclusion:A high frequency of TMD was observed in people with psychosocial disabilities and a high CPO-D index. Abstract Temporomandibular dysfunction (TMD), anxiety, and depression are disorders that, due to the current lifestyle, are affecting an increasing portion of the population. Investigating the prevalence of the symptoms of these disorders during the quarantine due to the coronavirus 2019 pandemic (COVID-19) is important to outline clinical strategies for patient care. Objective: This study assessed the prevalence of TMD symptoms, anxiety, depression, and oral behaviors and their associations during the social isolation due to COVID-19. Methodology: Questionnaires were used to assess TMD symptoms in accordance with the Diagnostic Criteria for Temporomandibular Disorders: clinical protocol and assessment instruments, a questionnaire to verify oral behaviors and Hospital Anxiety and Depression Scale to assess symptoms of anxiety and depression in students of dentistry at the Faculty of Health Sciences of the University of Brasília in May 2020. Qualitative data were subjected to descriptive statistics and chi-squared analysis (p<0.05). The relationship between quantitative and qualitative data was evaluated using Spearman's rho correlation (p<0.05). Results: There was a high prevalence of TMD symptoms, anxiety, and depression in the participants, resulting in association between gender and anxiety symptoms (p=0.029). There was a positive correlation between oral behaviors and TMD symptoms (r=0.364; p<0.001), between oral behaviors and anxiety symptoms (r=0.312; p=0.001), and between oral behaviors and symptoms of depression (r=0.216; p=0.021). Conclusion: Social isolation due to the COVID-19 pandemic has an impact on the prevalence of TMD symptoms, anxiety, and depression.

ABSTRACT

The aim of this study was to screen for painful TMD conditions by implementing the validated Axis I screening instrument from the Diagnostic Criteria for Temporomandibular Disorders. Using the screener as a surrogate, the prevalence of the conditions was estimated among a convenience sample of dental students in Peru. A total 2,562 dental students, 63.7% women, aged 18 to 62 completed the instrument. Prevalence was estimated using both the short and long versions. The prevalence of painful TMD conditions was 19.4% with the short and 16.1% with the long version. The distribution of the conditions according to gender differed significantly between groups (p<.001). Prevalence estimates of painful TMD conditions using the screening instrument seems to be logistically adequate in a field assessment involving multiple geographic and cultural regions in Peru. These estimates seem to be consistent with internationally reported values.


El objetivo de este estudio fue tener una estimación de las condiciones de los trastornos temporomandibulares asociados al dolor, mediante la implementación de un instrumento validado de triaje para la medición del Eje I de los Criterios Diagnósticos para los Trastornos Temporomandibulares (CD/TTM). Usando este instrumento como un sustituto, se estimó la prevalencia de estas condiciones en una muestra por conveniencia de estudiantes de Odontología en el Perú. Un total de 2,562 estudiantes de Odontología, 63.7% mujeres, entre las edades de 18 a 62 años de edad, completaron el instrumento. La prevalencia fue estimada empleando las versiones corta y larga del instrumento. La prevalencia de los TTM asociados al dolor fue de 19.4% con la versión corta y 16.1% con la versión larga. La distribución de estas condiciones fue estadísticamente diferente según el sexo (p<.001). El empleo del instrumento de triaje para estimar la prevalencia de los TTMs asociados al dolor parece ser logísticamente adecuado en un escenario de diversas regiones geográficas y culturales en el Perú. Estas estimaciones parecen ser consistentes con valores internacionalmente reportados.

Resumo O objetivo deste estudo foi estimar a prevalência de dificuldade na mastigação e fatores associados em adultos de 20 a 59 anos em Patos, PB, Nordeste, Brasil. Foi realizado um estudo transversal com uma amostra aleatória de 532 indivíduos. A dificuldade na mastigação foi avaliada por meio de pergunta sobre dificuldade causada por problemas com dentes ou dentadura. Foram realizadas análises descritiva, bivariada e multivariada por meio de regressão de Poisson. A prevalência de dificuldade na mastigação foi de 30,5%. Na análise multivariada, os fatores associados à dificuldade na mastigação foram: faixa etária, escolaridade, tempo desde a última consulta, perda dentária severa, ausência de dentição funcional, uso e necessidade de prótese dentária, dor de origem dental e sinais e sintomas de alterações na ATM. A magnitude das associações entre as variáveis, com destaque para perda dentária e necessidade de prótese dentária, reforça a importância deste indicador subjetivo na avaliação da condição de saúde bucal dos indivíduos adultos e mostra que a dificuldade na mastigação está associada a uma estrutura multidimensional de fatores.


Abstract The aim of this study was to estimate the prevalence of chewing difficulty and associated factors in adults aged 20 to 59 years in Patos, in the State of Paraiba in the Northeast Region of Brazil. A cross-sectional study was conducted with a random sample of 532 participants. The outcome chewing impairment was assessed using the question "How often do you have difficulty eating due to problems with your teeth or dentures?". Crude and adjusted prevalence ratios were estimated using Poisson regression. The prevalence of chewing difficulty was 30.5%. An association remained between the following variables and chewing difficulty in the final multivariate models: age group, schooling, being a smoker/non-smoker, length of time since last visit to the dentist, severe tooth loss, absence of functional dentitions, dental prosthesis use, need for a dental prosthesis, oral pain, and signs and symptoms of TMJ dysfunction. The magnitude of the associations between the variables and chewing difficulty, notably tooth loss and the need for a dental prosthesis, emphasize the importance of subjective indicators for assessing the oral health status of adults and shows that chewing difficulty is associated with a range of multi-dimensional factors.

Abstract Objective the aim of this study was to describe the frequency of psychosocial diagnoses in a large sample of patients attending a tertiary clinic for treatment of temporomandibular disorders (TMD). Material and Methods six hundred and ninety-one patients who sought treatment for pain-related TMD were selected. Chronic pain-related disability (Graded Chronic Pain Scale, GCPS), depression [Symptoms Checklist-90 (SCL-90) scale for depression, DEP] and somatization levels (SCL-90 scale for non-specific physical symptoms, SOM) were evaluated through the Research Diagnostic Criteria for TMD (RDC/TMD) Axis II psychosocial assessment; TMD diagnoses were based on the Axis I criteria. Results the majority of patients presented a low disability or no disability at all, with only a small portion of individuals showing a severely limiting, high disability pain-related impairment (4.3%). On the other hand, abnormal scores of depression and somatization were high, with almost half of the individuals having moderate-to-severe levels of depression and three-fourths presenting moderate-to-severe levels of somatization. The prevalence of high pain-related disability (GCPS grades III or IV), severe/moderate depression and somatization was 14.3%, 44% and 74.1% respectively. Gender differences in scores of SCL-DEP (p=0.031) and SCL-SOM (p=0.001) scales were signficant, with females presenting the highest percentage of abnormal values. Conclusion patients with TMD frequently present an emotional profile with low disability, high intensity pain-related impairment, and high to moderate levels of somatization and depression. Therefore, given the importance of psychosocial issues at the prognostic level, it is recommended that clinical trials on TMD treatment include an evaluation of patients' psychosocial profiles.

Abstract This article aims to analyze the prevalence and associated factors of TMJ alterations in elderly institutionalized. An cross-sectional study of 1192 elderly institutionalized in Brazil was undertaken. Intra and extra-oral exams were performed and the TMJ was evaluated based on the Oral Health Assessment Form of the WHO (1997). Demographic data and elderly dependence were also collected. The self-perception of the elderly regarding oral health was measured using the Geriatric Oral Health Assessment Index - GOHAI. Data analysis was performed using the Chi-squared test and robust Poisson regression (level of significance of 5%). The prevalence of TMJ alteration was low compared to adults and was only associated with the state of dependence of the elderly individual (independent had 45.4% more alterations than dependent individuals), gender (women had 47.4% more alterations), self-perception of oral health (who evaluated negatively had 65.6% more alterations) and the need of upper dentures (who needed some kind of upper dentures had 45.8% more alterations than those who did not). Despite being low, the presence of alterations in the TMJ was more frequent in elderly independent, women, who evaluated negatively oral health and need some kind of upper dentures.


Resumo O objetivo deste artigo é avaliar a prevalência e os fatores associados a alterações na ATM em idosos institucionalizados. Estudo observacional e transversal, conduzido em 1.192 idosos brasileiros. Foram realizados exames intra e extraoral, baseados na Ficha de Avaliação da OMS (1997). Dados demográficos e nível de dependência também foram coletados. A autopercepção do idoso quanto à saúde bucal foi obtida através do GOHAI (Geriatric Oral Health Assessment Index). Na análise dos dados foram usados o teste do Qui-quadrado e a Regressão robusta de Poisson (nível de significância de 5%). A presença de alguma alteração na ATM foi pouco prevalente quando comparada a dos adultos, e esteve associada à condição de dependência do idoso (os independentes tiveram 45,4% mais alterações do que os dependentes), ao sexo (mulheres tiveram 47,4% mais alterações), à autopercepção de saúde bucal (quem avaliou negativamente teve 65,6% mais alterações) e à necessidade de prótese (PT) superior (quem necessita de algum tipo de PT superior teve 45,8%). Apesar de ter sido baixa, a presença de alterações na ATM esteve mais frequente em idosos independentes, do sexo feminino, que avaliaram negativamente a saúde bucal e que necessitam de algum tipo de PT superior.

Objectives: To determine the prevalence of bruxism and TMD in two populations in Uruguay: the capital city, Montevideo, and outside Montevideo. Method: A cross-sectional study was conducted throughout the country with stratified multistage sampling. Results: The variables selected and analyzed in this first paper were: degree of mouth opening, muscle pain, TMJ clicking, parafunctional wear facets, headache, history of joint noise, feeling of clenching during sleep and self-perception of stress. The following global prevalence rates were found: TMD: 55%, with at least one symptom, it being more prevalent in Montevideo (57%) than outside Montevideo (53%); with at least one sign: 44%, it also being more prevalent in Montevideo (47%) than outside Montevideo (41%). As for active bruxism, regarding the feeling of clenching during sleep, a prevalence of 30.72% was found in Montevideo and 23.19% outside Montevideo, while being or having been a bruxer in the assessment of the parafunctional wear facets had a prevalence of 71.95% in Montevideo and 62.17% outside Montevideo. Conclusions: With this survey, we found a high prevalence of TMD and bruxism in the Uruguayan population and encourage the development of collective prevention and treatment actions.

La articulación temporomandibular (ATM), como todas las articulaciones, es afectada por enfermedades inflamatorias o degenerativas, traumatismos, malformaciones, infecciones, quistes y tumores. La artrosis u osteoartrosis (OA) es la enfermedad degenerativa más frecuente de las articulaciones del cuerpo humano, incluyendo la ATM. El propósito de este estudio fue determinar la prevalencia de signos imagenológicos de trastornos degenerativos en el proceso condilar de la ATM, presentes en radiografías panorámicas digitales (RPD) de población chilena. Para ello se realizó un estudio de corte transversal que utilizó 651 radiografías de individuos adultos, en las que se buscó la presencia de los siguientes signos imagenológicos de osteoartrosis: esclerosis, pérdida de la cortical, erosión, osteofitos y quistes endocondrales. La prevalencia de signos radiográficos de OA fue de un 35 % (n=230); donde el 50 % de los individuos con signos de OA fueron mayores de 50 años, mientras la diferencia se distribuyó de forma homogénea en los distintos rangos etáreos. El signo más prevalente fue el osteofito (25 %) y la pérdida de cortical fue la menos observada (3 %). Además, la esclerosis presentó una prevalencia del 6 %, la erosión del 4 % y los quistes endocondrales del 6 %. Por su parte la frecuencia de signos observada en hombres fue mayor, detectándose un aumento en relación directa con la edad independiente del sexo. La radiografía panorámica por su accesibilidad, baja dosis de radiación y constituir un examen de rutina, constituye un medio apropiado para pesquizar hallazgos de signos degenerativos de la ATM.


The temporomandibular joint (TMJ), like all joints, is affected by inflammatory or degenerative diseases, traumatisms, malformations, infections, cysts and tumors. Osteoarthrosis (OA) is the most common degenerative disease of the joints of the human body, including the temporomandibular joint. The purpose of this study was to estimate the prevalence of imaging signs of degenerative disorders in the TMJ condyle, present in digital panoramic radiographs in Chilean population. A cross-sectional study was conducted using 651 radiographs of adult individuals. The presence of the following imaging signs of osteoarthrosis was recorded: sclerosis, cortical loss, erosion, osteophytes and endochondral cysts. The prevalence of radiographic signs of OA was 35 % (n = 230); where 50 % of the individuals with signs of OA were older than 50 years, while the difference was distributed homogeneously in the different age ranges. The most prevalent sign was osteophyte (25 %) and cortical loss was the least observed (3 %). In addition, sclerosis presented a prevalence of 6 %, erosion of 4 % and endochondral cysts of 6 %. On the other hand, the frequency of signs observed in men was greater, detecting an increase in direct relation with age independent of sex. The data of this study allows us to conclude that the prevalence of OA in the studied population was 35 % with a higher representation in men and older people. Both the value of the prevalence of OA and that corresponding to each sign are affected by the modality of radiographic analysis and methodological considerations.

RESUMEN: El término "Transtornos Temporomandibulares" (TTM) incluye numerosos problemas clínicos asociados con las articulaciones temporomandibulares, músculos de la masticación y otras estructuras asociadas. El bruxismo, un hábito oral parafuncional, consiste en rechinar o apretar de manera rítmica e involuntaria los dientes, lo que puede conducir a trauma oclusal y problemas articulares. El objetivo del estudio fue determinar la frecuencia y prevalencia de signos y síntomas de TTM y bruxismo en niños escolares de 6 a 12 años, en una población de San Luis Potosí, México. Se empleó un diseño transversal descriptivo en una muestra de 314 participantes, estudiantes de una escuela primaria de la ciudad de San Luis Potosí, México. Los participantes fueron seleccionados aleatoriamente en forma estratificada y polietápica. La muestra total consistió en 153 femeninos y 161 masculinos. Los signos y síntomas de TTM más frecuentes fueron la alteración de la función de la ATM (35 %), seguido por los ruidos y chasquido articulares (29,2 %). En los sujetos con diagnóstico de bruxismo, la alteración más frecuente fueron molestias asociadas en la ATM (19,4 %), dolor de cabeza (17,8 %) y atrición dental (16,5 %).


ABSTRACT: The term "Temporomandibular disorders" (TTM) includes numerous clinical problems associated with joint temporomandibular, muscles of mastication and other associated structures. Bruxism, oral parafunctional habit, consists of grinding or clenching of involuntary, rhythmic manner, which can lead to trauma, occlusal and problems joint. The objective of the study was to determine the frequency and prevalence of signs and symptoms of DTM and bruxism in school children aged 6 to 12, in a population of San Luis Potosí, México. A crosssectional descriptive design was used in a sample of 314 participants, students of a primary school in the city of San Luis Potosi, Mexico. The participants were randomly selected in tiered form and conglomerated. The total sample consisted of 153 female and male 161. The signs and symptoms of TMD frequent were alteration of the function of the ATM (35 %), followed by noise and snap joint (29.2 %). In subjects with a diagnosis of bruxism, the most frequent alteration was associated with TMJ discomfort (19.4 %), headache (17.8 %) and dental attrition (16.5 %).

ABSTRACT

The aim of this study was to determine the prevalence of temporomandibular disorders ( TMDs) in a sample of Ecuadorian older adults in 2018. This is a cross-sectional study, which was conducted with a sample of 143 older adults of both sexes of the "Senderito de Amor" Elderly Home. The evaluation instrument was the Helkimo index, with the following parameters: presence of muscle pain, difficulty in opening and closing movement, joint pain, joint noises, limitation of mouth opening, limitation of lateral movement, and propulsion and deviation of the jaw. Overall, 88.8% of the population had TMDs, with 85.7% in the group of 60­70 years, 81.8% in the group of 71­80 years, 96.7% in those over 80 years, and 81.0% men and 92.1% women. Statistically significant differences were found by age (p = 0.035), but not by sex (p = 0.055). The most frequent signs and symptoms were joint noise (82.5%), followed by deviation of the jaw during opening (60.1%), reduction of the mouth opening (41.3%), difficulty of opening movements and closure (35.0%), reduction in movements of laterality and propulsion (29.4%), joint and muscle pain (24.5%), muscle pain (18.9%), and joint pain (16.8%). In conclusion, there is a high prevalence of temporomandibular disorders in older adults, and the most frequent signs and symptoms were joint sounds and deviation of the jaw during the mouth opening

Objective: To describe the clinical and sociodemographic characteristics of patients with temporomandibular joint ( TMJ) dysfunction at the University Hospital of Maracaibo. Methods: A retrospective study was conducted by assessing the clinical history of patients with TMJ dysfunction who underwent orthopedic treatment in the Oral Surgery Unit of the Dentistry Service of the University Hospital of Maracaibo. Information was collected on socio-demographic characteristics (age, gender, origin, and occupation) and clinical findings related to the presence, location, area, beginning, frequency, cause, and duration of pain, and signs present during mandibular dynamics; additionally, a structural and functional diagnosis was made. The data were processed and analyzed using SPSS v.9. Results: 221 patients were included, with only 10% of the disorders observed in males. Regarding occupation, 39.1% worked at home and 28.3% were students. 97.2% of patients presented pain, and of these, pain was localized in 75%. During mandibular movement with maximum opening, 47.4% showed some difficulty, which was more significant in males. TMJ noises were noted in 74.9%, more often in females. In addition, the previous partial displacement of a disk with recapture presented an incidence of 66.4%. Conclusion: The prevalence of TMJ dysfunction was higher in female patients and in adulthood. Noises were present with a high frequency, as was limited mandibular movement.

Introducción: El consumo de drogas puede constituir un agente etiológico de trastornos témporomandibulares (TTM), lo cual debe ser tenido en cuenta al momento del diagnóstico y en la elaboración de un plan de tratamiento. Objetivo: Estimar la prevalencia de signos y síntomas de TTM en una población con dependencia a las drogas al inicio del régimen de internación y analizar posibles factores asociados. Material y método:Fue un estudio descriptivo y transversal, que incluyó pacientes consecutivos al inicio del tratamiento por consumo problemático de drogas. Participaron 135 pacientes internados en el Portal Amarillo y examinados por un investigador calibrado. Se solicitó consentimiento informado al participante y/o su representante legal. Resultados: La prevalencia de participantes con uno o más signos actuales de TTM fue de 29,7% y con uno o más síntomas actuales de TTM fue de 68,8%. La prevalencia de síntomas actuales de TTM presentó asociación significativa con autopercepción de estrés (p=.03), consumo de mate (p=.03) y de alcohol (p=.03). Conclusiones: La prevalencia de síntomas de TTM en una población en tratamiento por dependencia a las drogas es elevada respecto a la población general. Esto debe ser tenido en cuenta a la hora de desarrollar políticas de prevención y tratamiento.


Introduction: Drug consumption may be an etiological agent of temporomandibular disorders (TMD) which should be considered when diagnosing a patient and developing a treatment plan. Objective: To estimate the prevalence of TMD signs and symptoms in a drug-dependent population at the beginning of their admission, and to analyze possible associated factors. Materials and methods: This was a descriptive cross-sectional study that included patients at the beginning of treatment for problematic drug use. We studied 135 subjects admitted to Portal Amarillo, Uruguay. The patients were examined by a calibrated researcher. Informed consent was requested from each participant and/or their legal representative. Results: The prevalence of subjects with one or more current TMD signs was 29.7%, and those with one or more current TMD symptoms accounted for 68.8%. The prevalence of current TMD symptoms showed a significant association with self-perceived stress (p=.03), mate consumption (p=.03) and alcohol consumption (p=.03). Conclusions: The prevalence of current TMD symptoms in the population under treatment for drug dependence is high compared to the general population. This should be considered when developing prevention and therapeutic strategies.
